The people who call Dr. Joanna R. Swauger, Dr. Lauren H. Ayersman, and Deanna Nemec, CRNP, their medical team know that they provide care that is respectful of and responsive to their individual preferences, needs and values.

Dr. Joanna R. Swauger, Dr. Lauren H. Ayersman, and Deanna Nemec, CRNP received recognition from the National Committee for Quality Assurance for their patient-centered coordinated care.

The National Committee for Quality Assurance (NCQA), a private, non-profit organization dedicated to improving health care quality, also recognizes that these medical professionals ensure that the values of each patient guide their clinical decisions. The NCQA recently recognized Mon-Vale Primary Care Practices, Inc. (Swauger/Ayersman/Nemec) as a Level-3 Patient Centered Medical Home. The organization commended the practice on “Achievement of Recognition for Systematic use of Patient-Centered Coordinated Care Management Processes.”

“Medical homes lead to higher quality and lower costs,” said Susan M. Flynn, chief executive officer of Vale-U-Health, which oversees Mon-Vale Primary Care Practices, Inc. “In addition, medical homes can improve patients’ and providers’ experiences of care.”

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that Mon-Vale Primary Care Practices, Inc. (Swauger/Ayersman/Nemec) has the tools, systems and resources to provide its patients with the right care, at the right time.”

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
